Residential Care Facilities (RCF) Subsidy Program: prevents homelessness by providing a subsidy for safe and affordable supported housing in a communal setting (Residential Care Facility) for people who require assistance with the daily activities of life.

24-hour emergency shelter providing 21 beds for youth between 16 and 21 years of age. Helping youth who are street-involved or homeless due to family violence, conflict, abuse, neglect, poverty and exposure to mental health and addiction issues.

Community Outreach Service - Help with getting back into school or finding employment, Assistance obtaining or replacing ID, Access to housing workers, Help with applying for or maintaining social assistance, Hot meal program, seven days a week (lunch and dinner), Ontario Works Youth Trustee Program, Connections to community supports, Free laundry facilities and clothing/household donations, Access to all on-site mental health, medical care, addiction/substance use services, Recreational and life skills programming, Early diversion and family mediation.

Housing focused shelter for families experiencing homelessness in the Hamilton area. All of our services and supports are focused on helping families secure housing in a timely manner. Shelter diversion and eviction prevention services are also available to families that qualify. Our specialized housing staff work with each family to create a housing plan with a 30-day goal.

Case Managers will support the youth (16-25 years) by attending apartment viewings, building relationships with landlords and facilitating with Ontario works and Ontario Disabilities Support Project.

The Homelessness Prevention Case Manager (HPCM) works with street involved and homeless youth.

The Intensive Community Case Manager (ICCM) works with youth experiencing chronic or episodic homelessness.

The Housing Engagement Support Worker (HESW) worked alongside the ICCM, co-ordinate key tasks that are required for a youth to successfully transition from experiencing homelessness into safe and stable housing.

COAST - Crisis Outreach and Support Team is a partnership between Mental Health Workers at St. Joseph’s Healthcare and specially-trained officers of the Hamilton Police Service. The program serves the residents of the City of Hamilton who have serious mental health issues and are in crisis, including a 24/7 crisis line.

Mobile Crisis Rapid Response Team (MCRRT): First Responders consisting of mental health professionals and dedicated uniformed police officers in a marked police cruiser. This team can respond to 911 calls for people in crisis, from 10:00 am until 1:00 am daily. Expert mental health assessment and support can get to a person in crisis within minutes.

*Public cannot request MCRRT - the team is dispatched by Police Services as needed.
